I have focused my research on the clinical aspects of type 2 diabetes particularly as regards investigating novel agents and other strategies for the prevention and treatment of diabetes and pre-diabetes. As part of the Center for Metabolic Research at the VA Hospital, I participate in several studies which utilize the hyperinsulinemic and hyperglycemic clamp studies and muscle and fat biopsies to evaluate insulin sensitivity and insulin secretion in both non-diabetic and diabetic subjects, before and after various therapeutic interventions. I am currently a Co-investigator in the NIH/NIDDK sponsored multicenter Diabetes Prevention Program Outcome Study, “DPPOS”. This study is a continuation of the earlier landmark diabetes study that showed the intensive lifestyle modification with diet/exercise or intervention with metformin significantly decreased the incidence of diabetes in patients with pre-diabetes. Currently, the study is evaluating the incidence and progression of diabetic complications, carotid intimal media thickness, abdominal fat distribution and cardiovascular disease in these pre-diabetes and early diabetic subjects. I am also a Co-Investigator on the NIH sponsored GRADE study (Glycemia Reduction Approaches in Diabetes: A Comparative Effectiveness Study) which is a randomized clinical trial in patients with recent-onset (<5 years duration) type 2 diabetes and will compare the metabolic effects of four common anti-diabetic drugs when combined with metformin. I am also the PI on several industry sponsored grants, evaluating the efficacy and safety of medications for the treatment of type 2 diabetes and its cardiovascular complications.
